Application

Please read How to get support first.
The application should provide the following information:
- Name of the organisation / hospital (with postal address and (if available) email address
- Your name and relationship to the above-mentioned organisation
- Brief description of the project (not more than 150 words!!)
- What support is necessary (know-how, equipment, software etc)?
- Will technical advice be required? If so, what kind (computer, mechanical, electrical etc)?
- What structures are already available in the organisation which could help the project in its objectives e.g. personnel, organisational, technical, buildings etc?
- The estimated cost of the project
- Which person would be responsible for carrying out the project?
- Has support already been applied for from other sources? If so, from which? Were these applications successful?
- Where did you first hear about the KAMARUTA Club?
